employee was cleaning the rounder bars from operator side being on the step stool when he noticed something all the way on the other end so employee went on top of the step stool (top layer not a step) and try to see what was there when he was leaning toward the other side the step stool went tilting to the left causing the employee to go down with his left foot stuck on the top layer and coming down too fast on his right foot getting twisted and pulling on his left leg
