Before the incident
Dumping a loaded roll off container at the landfill.
What happened
The employee was operating a roll off truck (Unit 1). He was at the [REDACTED] to dump the loaded roll off container. Another roll off driver (Unit 2) was dumping his roll off container at the [REDACTED] as well. Unit 2 driver had emptied his roll off container but a large metal door had gotten lodged inside the roll off container. Unit 2 driver had lowered the rack of Unit 2 and dislodged the large metal door. Then the driver of Unit 2 began to raise the roll off container to get the large metal door out. At this time the injured employee states he seen the handle that secures the door of the roll off container swing out towards the ground. The injured employee stated that he attempted to grab the handle to prevent damage to it and at that time the large metal door slid out of the roll off container and after it hit the ground the large metal door fell back and struck the employee in the right ankle leg.
