Before the incident
Employee was pouring the cooking oil into the twin arm mixer. The oil was packed on totes organized on a rack there are shelves of two totes one of top of the other. When employee when to pick up a tote with oil by mistake the employee instead of lifting the tote employee dragged the tote which caused that the tote on the bottom fell on the ground spilling the oil of the container and the lid of the container also fell on the ground. The employee step on the lid that fell causing him to slide and fall on the ground. Employee stated that he hurt his right side including his right hip tight and arm.
What happened
