What happened
EE was removing fish out of a tote that was sitting on a pallet jack. A TM walked up to the tote and pulled the handle of the pallet jack to lower the tote not realizing the EEs foot was under it. when the tote came down it landed on the EEs Left big toe causing the toenail to be lifted away from the skin.
