[REDACTED] Cousins was using a syringe to fill a small bottle with THF compound from a larger bottle holding the compound. The syringe slipped out of his hand and hit one of the bottles. The compound in the syringe shot the THF compound into his right eye. [REDACTED] was not wearing company-required safety glasses during the incident. His eye started to burn and was a little blurry. He used eye wash from the first aid kit to rinse his eye. [REDACTED] is having his eye examined at Med Express in [REDACTED] Claim No: C224-0371378
