Before the incident
responding to breakdown on [REDACTED] floop
What happened
Employee responded to a breakdown call at the K4 Floop at the beginning of his shift. The strip had moved over the edge of the flanged roll as shown in attachment floop1. [REDACTED] loosened the shaft collars and attempted to move the strip back into place by hammering the roll over on the shaft towards the inside of the floop. As employee contacted the roll with the hammer he felt something hit him in the eye. He confirmed he was wearing his safety glasses. The regular ones from the vending machine. He does not know at the time what went into his eye as it was on the opposite side of where he was hammering.
