What happened
Repairing a welder on [REDACTED] (the actual welder). The welder itself has 2 fans that blow out on the back side. Employee installed a fan (to replace the old one that was no longer working) and he assumed the fan would blow out like the previous one but it was actually sucking air in. He put his hand up by the fan (inch or 2 away) to see if it was working and sucked his finger into the fan blade.
