What happened
During treatment session with bariatric patient (159kg) a Beasy board was trialed for slide board transfers. Patient successfully transferred from bed to bedside commode. Attempted slide board transfer from bedside commode to chair scale due to patients ' daily weight being needed. Patient began transfer with OT behind and PT in front for assist. As the transfer progressed therapists realized the chair scale was sliding despite it being in the parked setting. Patient began to state that she was unable to slide anymore therapists attempted to assist without success. Equipment continued to slide and OT tried to stabilize to prevent patient from falling. OT started to support an increased amount of patient 's weight at lower back while trying to maintain equipment position. PT called out for help and RN came to assist with [REDACTED] set up. OT informed PT that if patients ' weight was not offloaded quickly the equipment was at high risk of falling. OT continued to support patient 's weig
