What happened

On [REDACTED] at around [REDACTED] an IC Athlete grabbed a box Containing 60 + units from [REDACTED] location in the [REDACTED] and when she lifted the box she heard a pop under armpit. The athlete stated that she tried to work through it but it started hurting too bad. At approximately [REDACTED] [REDACTED] the athlete reported the injury to the Ops Manager who added the report into Process Maps. Once finished the athlete then connects with the Night Shift Safety to get a [REDACTED] Employer Authorization form. Once the form was complete the athlete left for [REDACTED] at [REDACTED] [REDACTED]. At [REDACTED] the athlete returned to work stating that [REDACTED] is closed due to [REDACTED] so the Athlete and Safety attempted a Telemed Visit in [REDACTED] B at [REDACTED]. After waiting for 11 mins. at [REDACTED] the athlete decided to cancel the Telemed visit and stated that she will go home and will go to [REDACTED] once they reopen.
