Preparing to unload a tanker of cocoa butter
What happened
Employee attempted to stop a small leak from a valve at the unloading port of the tanker and in the process loosened the valve which resulted in the valve unintentionally being released from the manifold resulting in the release of the entire contents of the tanker. The employee then attempted to reinstall the valve as the cocoa butter was discharging from the tanker and was subsequently sprayed in the eyes.
