Before the incident
On [REDACTED] an RSE was finishing his break and instead of going around an open trench that was 5ft deep and 3ft wide he tried to jump over it and he fell in striking his head. The RSE was then stuck in between the pipe and side wall. The customer and other RSE s on site called [REDACTED] and then contacted the Branch Manager. The RSE that fell into the hole was transported to the ER via Ambulance and was admitted for observation. The RSE was released the following day. The RSE stated that he was just trying to get over to his relief and knew he shouldn t of tried to jump over the trench. His foot slipped off the edge and he fell into the hole striking his head on the pipe in the ditch. RSE was taken to Paoli Hospital [REDACTED] [REDACTED]
