Untying the spout of a reusable open-top bulk-bag that contained the raw materials (i.e. various rocks and sand) that was being dumped into a mixer. 2 or the 4 lifting straps broke while the bag was suspended (~1 foot) above the mixer. We have already implemented a corrective action of a remote pull string to open the spout and to remove ourselves from harms way.

2 of the 4 lifting straps simultaneously broke as the bag was suspended about 1 foot above the mixer frame. We have already eliminated the hazard however what was interesting about this is all 4 straps are inspected prior to being filled and again all 4 straps are inspected prior to being attached to the hoist.

Contusion right upper arm

The injury was caused by the weight of the bulk-bag pinching employees arm between the bulk-bag and the mixer frame.
