What happened
The large ring gear (~200 lbs) was not slid onto the shaft evenly so the team was using pry bars to try to properly align the ring gear on the shaft. [REDACTED] was pushing down on curled end of the pry bar and the pry bar slipped the flat end popped up and hit the Employee in one of his front teeth.
