Before the incident
The operator EE and EE were chaining the [REDACTED] digger to the international flat bed ([REDACTED]) to assist pulling out of the ditch where the digger had been backed into incase it needed assistance getting out of the ditch. The chain was already attached to the flat bed ([REDACTED]) by EE and EE was attaching the chain to the digger (S115003) . EE was not aware they were chaining to assist coming out of the ditch so he went to pull the flat bed out of the way. When he did this EE was under the digger and had just finished attaching the chain to it.
What happened
When EE pulled the flat bed forward it caused the chain to tighten and the chain was under EE this caused EE s body to lift forcing his upper body into the under carriage of the digger(S115003). [REDACTED] was waving and yelling for [REDACTED] to stop as soon as he started to move truck fwd. I EE had just gotten off a phone call with the other crew in the field arranging a truck swap and crew swap for the [REDACTED]. I saw the accident happen as I was walking back to the work sight. I ran to [REDACTED] and ask him If he was ok he was knelling with his head down. He looked up I noticed he had blood on his right cheek and had a cut under his eye. We retrieved a first aide kit cleaned him up. He stated at that time he felt fine but sore where cut was. We were getting ready to leave and that is when he started to feel dizzy and nausea. Called to report the incident then we headed to the hospital less then 5 min I got a call back then reported that we were on our way to the hospital and bri
