Broaching key: after the broach was flush with die the colleague was holding pin in place with fingers to get extra travel out of the broach. He was operating the hydraulic press and pushed the button to lower the cylinder.

The associate did not realize the clearance for his finger was gone when the cylinder lowered and it pinched his right index finger.
