ON [REDACTED] AT [REDACTED]. I WAS AT [REDACTED] DELIVERING A MAIL AND A PACKAGE. AFTER DELIVERING A PACKAGE TO THE DOORSTEP I WAS WALKING BACK TO THE TRUCK AN MY SHOE CAUGHT THE UNEVEN PORTION OF THE SIDEWALK. I FELL FORWARD AND HIT MY HEAD. AMBULANCE AND MANAGEMENT WAS NOTIFIED. I WAS TRANSPORTEDTO THE HOSPITAL.
