What happened
Employee was working on APBS an unsecured pallet hit her on the head. I had a call from APBS Machine. When I walked over I saw the employee sitting there and she explained to me that the pallet fell out and hit her on the head. I looked and noticed that the loader did not put the cross bar in place that would secure the pallet. Unfortunately the pallet fell out and hit the employee on the head.
