loading on machine

On [REDACTED] at approximately [REDACTED] mail handler was loading a pallet of scf bundles into the dumper on machine two. Her palletizer was facing the machine. Employee states that she took a step back and tripped on the forks of the lift which caused her to fall back onto the bar of the pallet lift. When i arrived at machine 2 mail handler was sitting on the floor and blood was on the floor. There was plastic wrap and straps from the bundles on the floor near the lift. She did have an open wound near the nape of her neck
