What happened
FTR [REDACTED] reported that while a FTR MH was offloading contractor trailer [REDACTED] with a double wide powered pallet lift the trailer moved forward causing the pit equipment to be half on the truck and half on the dock plate. Employee reported that the driver pulled in the doorway and then went to the bathroom. While he was in the restroom the truck moved forward and that there were no wheel chocks in front of his wheels. There were wheel chocks available for use and the driver did not hang keys as instructed
