What happened
On [REDACTED] I was out for delivery at around [REDACTED] I had dropped a package in the back of the truck and proceeded to reach down to pick it up while lifting my head back up I had hit it on the edge of a metal shelf in the back of my truck. This had caused the top of my head to split open and an open wound was formed on my head. I then proceeded to go to an urgent care after clocking out and was diagnosed with a concussion.
