What happened
On [REDACTED] at approximately [REDACTED] CCA reported to Management he was involved in a motor vehicle accident while he was traveling west bound on [REDACTED] when customer driving a dark grey van was traveling east bound turned left from the turning right lane in front of FFV. Police gave customer a citation. Carrier was alert and stated his head and hip was in pain. Carrier was transported to St. Lukes Springwoods Village Hospital to seek medical attention.
