SLIP TRIP FALL - RCA stepped out of the [REDACTED] delivery vehicle at [REDACTED] stepping onto black ice. She slipped and fell backwards flat onto her back. Medical sought. The carrier was wearing [REDACTED]. the apartment complex stated that they had placed out ice melt in the area but according to the carrier and the investigating supervisor it could not be detected. The area where the carrier fell is slate rock material. A RIMS alert has been added for the future notifying all carriers who carry this route to be cautious during days with inclement weather of the potential for slips and falls.
