On [REDACTED] at about [REDACTED] Carrier reported to Management he hurt his lower back while delivering a heavy package to customer door. Upon completion of his assignment carrier stated he did not want to go to the doctor to seek medical attention. Carrier wrote his statement on a Form 0-13 at [REDACTED] on [REDACTED]. [REDACTED] carrier returned at [REDACTED] on [REDACTED] with a signed form 13 and stated he now needed medical attention. Management instructed carrier to go to a medical provider on [REDACTED] however carrier went home for the day. On [REDACTED] during delivery carrier returned to the station requesting documentation to seek medical attention.
