What happened
Employee 's statement on the claim form indicates he was pushing tray cart near the machines when one of the wheels stopped suddenly. Employee claims this caused pain and arms and legs went numb. The employee did not report an incident to management the employee called for a member of management to the [REDACTED] computer to request a leave slip when asked what’s going on he stated he had to go. I asked if anything happened and he wouldn’t give me an answer. I asked if he needed a break he said no. He repeated he had to go; a break wouldn’t be any good. He would not provide any detail or a statement. He walked off towards [REDACTED] when I was speaking with him I called him back and he said he was going to get his bag and water then he was leaving. He returned filled out a leave slip and left. I talked to the other clerk who was working with him she reported that he was loading mail and it went out of sequence and he walked off the machine.
