What happened
The employee was moving an order picker down the picking aisle NF. A wooden crate on the warehouse rack was sticking out into the aisle and the order picker collided with it. The employee applied the brakes on the order picker causing the crate and glass on the back of the order picker to shift forward striking the employee in the back.
