Before the incident
Employee gathered information and tools needed for skimming any debris from the top water of the [REDACTED].
What happened
Employee was completing normal cleaning of the weir dam with the pool skimmer. As they were standing on the second set of grates that cover the weir dam the second grate in from the parking lot slid from its resting point and out of it hold down clamps falling into the water. As the employee went to catch themselves their hand landed on a sharp object that punctured the palm of the right hand.
Injury or illness
Puncture to the right center of palm Strained right shoulder. Employee had future follow up appointments from pain in right shoulder on 10 26 23.
Object or substance involved
The grate the employee was standing on slid out of the clamp hold downs. This action caused one side of the grate to fall. Employee fell and reached out to stop his fall injuring their hand and shoulder.
