Before the incident
The injured employee removed the escape slide from the machine and took it back to the maintenance shop. The escape slides connection nut that holds the escape slide to the escape cylinder rod was stripped out and needed to be cut off and a new connection nut needed to be welded back on.
What happened
The employee attempted to remove this connection nut by holding the escape slide with a pair of pliers and pushing it into the vertical bandsaw blade. The escape slide became unsecured and the momentum caused the employee to snag his left index finger on the vertical bandsaw blade.
