Before the incident
Walking stepped into a trench.
What happened
The employee was performing a typical and repetitive task of using a [REDACTED] vehicle to remove empty and replace a cullet buggy in the [REDACTED] basement area. The operator backed the [REDACTED] and buggy into a load position potentially and unknowingly dislodging a section of trench grating with the vehicle buggy wheels exposing the hot cullet water flowing in the trench. Due to heavy steam in the area the employee did not see that the grate (14”x14”) was out of position and after exiting the vehicle and taking a few steps he stepped into the trench containing hot cullet water. The employee was wearing the designated boots (6” high leather) for the task. He reported the incident immediately and received treatment at LMH ER and OSU ER for 1st and second degree burns on his foot ankle.
