Employee just finished cutting a bundle of 3x2 tube on the hyd-mech saw. He then looped a chain from the crane around one end of the bundle and lifted it with the crane to drain coolant from the bundle.

Once bundle was lifted employee grabbed a 4x4 to put under the bundle. As his hand was under the bundle the chain slipped off of the parts causing the bundle to fall on his left hand index finger.
