What happened
Patient confused attempting to leave. Primary RN attempted to reorient patient. Patient afraid and thinks staff are intruders in his home. Patient tried to exit room. Primary RN attempted hold patient with gait belt to prevent fall. Patient became agitat ed and turned and grabbed primary RN 's right wrist and twisted. Patient attempted to shove RN but RN was able to step back and release from patient 's grip. Called for help and security was called. Male physical therapist was nearby and came to assist whil e staff waited for security. Violent restraints placed on all four limbs. RN experiencing pain with movement of wrist swelling. Ice applied.
