Before the incident
At approximately [REDACTED] on [REDACTED] [REDACTED] [REDACTED] [REDACTED] [REDACTED] was cleaning equipment and parts for the bologna system in manufacturing. While cleaning pump parts for the transfer hopper at approximately [REDACTED] a [REDACTED] pump rotor fell from the rack onto the top of his right foot causing injury. He reported this to Sanitation Manager [REDACTED] at approximately [REDACTED]. At that time when [REDACTED] asked [REDACTED] if he was ok he stated that he was. [REDACTED] saw plant nurse [REDACTED] prior to leaving at approximately [REDACTED]. He was evaluated found to have a swollen foot and was treated. [REDACTED] sent him to Concentra to be evaluated further.
