Before the incident
The maintenance team were removing the wheels from the head on the [REDACTED] plannisher ([REDACTED]) in preparation for wear evaluation and possible repair The machine was lockedout and a take5 worksheet had been completed prior to work The team member was wearing cut resistant gloves While removing the upper plannish wheel there was some difficulty removing it and the insert that holds it It is suspected that the grease lubricating the assembly was sticky enough to cause removal to be more difficult than expected The team member placed his hands on both sides of the assembly and applied increasing force in his attempt to remove it at which time the assembly came loose from the head unit The sudden release caught him off guard and his hands with the assembly dropped approximately 68 resulting in his left pinky finger being impacted between the wheel assembly and the lower wheel bracket below it on the plannisher arm The resulting impact caused pain but no bleeding The incident was rep
