On [REDACTED] EE reported back injury pain that started approx. 2 weeks prior to reporting. ([REDACTED]) EE states he did not report initially because he just assumed it was normal pain and self treated himself by apply Heat Ice and stretched daily. EE reported injury today ([REDACTED]) because he felt nerve pain in his lower back and on left leg. At this time work has been missed. EE left before end of shift on [REDACTED]. EE will contact VAT on [REDACTED] and will follow up with Concentra on [REDACTED].
