What happened
EE used forklift to stack 18 inch pipe on truck and during the loading process one pipe was stacked crooked. The EE stepped onto the truck bed and manually lifted the pipe overhead to re-orient the pipe. The pipe slid back onto the rear of the EE 's head and neck.
