Repairing a vacuum line on a piece of machinery that provides plastic material to a pulverizer
What happened
A cloth tie wrap was thrown up to an the employee to raise a vacuum line. Employee grabbed the line and wrapped part of the material around fingers as he was pulling the trailing material up. The trailing material slid in to a small opening where the drive shaft of an electrical motor (Approx. 10000 RPM). The shaft caught the material and pulled it around the shaft. Essentially removind two fingers from the employee
