What happened
A Team Lead was packing a customer order and took their safety gloves off to fix a jam in the water-activated tape machine. After the jam was cleared the employee continued working but forgot to put their safety gloves back on. The employee proceeded to form the bottom of a cardboard box that was considered defective. As the employee was applying the tape to the bottom of the cardboard box their thumb slid across the cardboard causing a deep laceration.
