Working in the Maintenace area

At [REDACTED] during a scheduled maintenance on 2nd shift of [REDACTED] [REDACTED] the front of the [REDACTED] truck [REDACTED] was being lifted by another [REDACTED] truck #[REDACTED] using a strap attached between [REDACTED] 's mast and #[REDACTED] 's forks. Blocks were used to secure the [REDACTED] from dropping but the blocks were removed so the [REDACTED] could be lowered for the wheels to be slid on. Immediately after the blocks were removed the strap broke causing the [REDACTED] [REDACTED] to drop hitting the maintenance technician 's left foot ankle. The employee 's left foot was in line of fire which resulted in the employee 's foot being struck by the mast. Employee was wearing safety steel toes and other required PPE. Employee was taken to the Emergency Room and 3 fractures were found on their foot ankle. Employee went home after his foot was imobilized and is scheduled for further outpatient visit.
