Unloading participant in wheelchair from back of agency vehicle.

After unloading participant from the agency vehicle employee lifted the ramp and folded in into place not realizing the ramp was not locked in place. Then proceeded to bend down to lift the lower portion of the ramp and while doing so the upper portion ramp went forward and struck employee top of head.

When the upper portion of the ramp came out it struck the employee on the top of head causing a small laceration to the right ear. The ramp also struck the employees right shoulder blade which caused a contusion to part of head strain to muscle at neck level and laceration to right ear.

While bending down the van lift came undone out of place and struck employee on top of the head causing a small cut to right ear and for the employee to be hit.
