What happened
When the employee returned from break the employee running the equipment while she was on break notified her that a button on the machine was loose and to keep an eye on it. The employee noticed something coming out of one of the ports. She thought it was the button and reached in to pull it out without turning off the equipment first. As she did so the knife made a rotation and cut the tip of her right index finger.
