[REDACTED] [REDACTED] in [REDACTED] HE ship prep Two employees were working on the ship prep procedures for 254148 Beamline procedure E80003240 Beamline Ship Prep page M14 E14 They had the cryo pump E11737500 strapped to the crane the two bottom claws were removed and reinstalled with the flat side facing the cryo to support the weight The rest of the claws were removed the injured employee was supporting the head of the cryo pump The crane was operated by the witness they operated the crane upward slowly tension developed on the strap and the cryo did not release One of the claws was still attached to the cryo pump in the lower right around the [REDACTED] position The claw snapped under the pressure causing the cryo pump to swing into the employees mouth The employee chipped a front tooth they received cuts inside their mouth They walked off the floor was noticed by the lead The two talked gave him ice after 1520 minutes he employee left work to ice his mouth They are going to the den
