team member responded to a cap fault on line 4 that was causing bottles to back up to the filler. The team member rushed over to stop the line in order to clear the cap fault and bottle jam.

When the team member reached the epoxy portion of the floor by the filler outfeed they slipped from water and debris buildup and fell backwards and caught left forearm on metal shelf that covers the aerator causing a laceration.
