Before the incident
team member began preparing for preform changeover on line 2. As TM opened door beside preform oven to clear out old preforms they bypassed the safety switch and turned blower back on to run out preforms into the scrap preform bin. One of the preforms became lodged and TM reached in to dislodge the preform. After struggling to dislodge the preform TM then placed other(left) hand inside cabinet to gain leverage.
What happened
When the preform was dislodged the TMs middle finger on their left hand became stuck in infeed star wheel. This caused deep lacerations on both sides of the finger as well as a fracture of the bone at the tip of the finger.
