Blade change out
What happened
EE was performing a blade change out task at the time of the incident. The blade being changed out was bent due to improper alignment of the bushing and the blade mechanism and blades are not normally bent when they are changed out. The blade change out requires EE to use L-shaped allen wrench to loosen two bolts on the blade fixture and replace the used blade with a new blade. Employee was not wearing cut resistant gloves at the time of the incident.
