What happened

Partner told his line lead that he injured his back during his shift yesterday. He stated that at the time he did not feel any pain until he woke up to come to work for his shift [REDACTED] and started to feel pain in his back. I am not sure what time this happened or what he was doing when this occurred. He stated that he reported this to supervision [REDACTED] but this supervision was working in a position at the time helping with breaks. He reported this around [REDACTED] [REDACTED] and He came into the caf� to let me know that he was leaving for the day due to his back feeling pain. He Denied AMR and stated that he plan on going home to aide his own back and plan to return to work on his next schedule shift. I gave him an 801 form anyway and let him know to fill it out if he decides to change his mind to go see a doctor. He also stated that he was going to call HR in the morning pertaining to his incident.
