Associate on Lift truck [REDACTED] was pulling rolls for cut order.
What happened
Lift [REDACTED] [REDACTED] was traveling North on [REDACTED] aisle crossing [REDACTED]. Lift [REDACTED] [REDACTED] was traveling South on [REDACTED] aisle. Lift [REDACTED] [REDACTED] passed by stationary [REDACTED] 3356 and made contact with their pole to the front left wheel of [REDACTED] [REDACTED]. The [REDACTED] operator on lift [REDACTED] stated that her forehead s[REDACTED] the lexan windshield on her [REDACTED] upon impact. Observed a two to two and half inch knot with abrasion bleeding occurred above the associates left eyebrow. The nurse was contacted and spoke with the associate directing first aid care of icing on and off and taking of over the counter medication until the nurse could arrive onsite for further evaluation. Lift [REDACTED]s [REDACTED] and [REDACTED] were taken to maintenance to be inspected prior to being put back into operation if possible.
