Before the incident
The employee was moving a standup forklift from the short corridor to the long corridor. Also another employee had just finished mopping the blue zone just outside the rollup door for the short corridor.
What happened
As the employee was exiting the short corridor into the blue zone the forklift would not turn but continued straight because the floor was wet. The lift was going to hit a metal barrier so the employee stuck his foot out to try and stop the lift. His foot was crushed between the barrier and the forklift.
