What happened
Employee was returning plastic wrap to the receiving desk after wrapping pallets and tripped on rubber floor mat. Employee landed on knees and then face down. First aid was provided by Walmart employees and then employee went to Chadron clinic.

Scrapes and burning to right and left palm of hands. Left and right knee burning and sharp pain when pressure applied. Right knee had bleeding laceration.
