The employee was attempting to check a possible carbon black leak on reactor #4.

While investigating the leak the employee walked to the second deck and started traveling towards the leak he stepped onto a grating that immediately twisted and broke. His left leg went in between the floor opening and the edge of the grating scraped his leg but did not pierce the uniform clothing. Afterward the employee right ankle twisted and he fell to the grating floor and landed on his right knee. The employee then got off the grating floor walked to the control room and notified his immediate supervisor. The employee cleaned up the impacted area with scrape spray peroxide and bandaged the scrape with material from the first-aid cabinet. The employee has minor pain in his right ankle a minor scrape on his right knee and a minor abrasion on his left shin. The employee has denied medical treatment at this time.
