Bolting up a 8 inch 150 gate valve

What happened

Turner employees were preparing to install 6 inch piping and new 1 inch suction spool on header at the number 2 HDS unit located in the [REDACTED] process area While the 12 inch spool was on the ground with the 8 inch olet in the upright position the employees installed the 8 inch 150 gate valve on the flange and completed tightening the bolts The 12inch flanges on the spool were cribbed with 4 inch X 4 inch timbers that were placed parallel to the spool piping After the valve was lifted with a forklift nylon sling and secured with bolts the sling was removed which left the spool totally stabilized by the cribbing As the injured employee turned to walk away from the spool the weight of the 8 inch valve upward position caused the spool to roll striking the employee on the right lower leg causing his lower leg to be pinned between the valve and the ground
