Employee was inside the wig wag machine removing stuck rubber off the flapper arm
What happened
njured employee was on Mill 2 and noticed Mill 3 was having trouble. Employee went over to assist in getting rubber off the flapper arms of the wig wag machine. After review of the camera footage it showed the employee climb up into the wig wag and on top of the pallet and was trying to cut rubber with the mill knife off the flapper arms. The shift supervisor was standing in that location watching the employee perform this task. The Mill 3 [REDACTED] was also trying to assist and getting the line back up and going. At the point before the incident the Mill 3 operator was told to go double tap the foot pedal to try to get the rubber unstuck.
